Consent

By signing a consent form, the participant or parent/guardian (if the participant is under 18) grants FROM permission to photograph, record, and share their story for publicity, marketing, and storytelling purposes.

 

Use of Photographs and Stories

• Minors 18 and Under: Photos and stories of minors will only be used for marketing or storytelling for two years from the date of first release. FROM reserves the right to use photographs after this for major historical/anniversary celebrations.

• Adults 18 and Over: Photos and stories of adults will only be used for marketing or storytelling for five years from the date of first release. FROM reserves the right to use photographs after this for major historical/anniversary celebrations.

 

Voluntary Participation

Participation is completely voluntary. By signing participant understands all materials will remain property of FROM and no gifts, services, or other incentives will be provided in exchange for consent. By signing, participants affirm that they are releasing their story, photos, and recordings on their own volition.

 

Rights and Withdrawal

Participants or parents/guardians for minors may withdraw consent at any time in writing.

FROM will make reasonable efforts to honor withdrawal requests, but cannot guarantee the removal of materials already published. FROM’s mailing address is: 11535 Fulton St E Lowell, MI 49331 and email is info@fromlowell.org.
